We’ve listened to your feedback. And now, we have some great news about our Customer Declaration form.


You can now access a new, stand-alone form for your client to sign at the beginning of the application. This means that you can now upload their signature, along with the initial documents, when you submit a case to us.  You won’t need to send the declaration form again when the application is completed either.

Before you send us the Customer Declaration form


You need to:


Enter the customer's name and application number reference (if you have it)


Make sure the customer has read and signed the declaration form


• Log in and upload via Click


NB: You won't need to include this form with the application if the client has already completed a stand-alone form – or if they're just changing the way they pay product fees.

Calls to 0845 and 0870 numbers cost 3p per minute, plus your phone company’s access charge. Calls to 0844 and 0843 numbers cost 7p per minute, plus your phone company’s access charge. Calls may be monitored or recorded for security and training purposes.
